Dear Valued Patient, 

We are writing to inform you of an important change to the way our practice will operate going forward. 

Immediate Transition to Telemedicine 

Effective immediately, our practice is moving to all telemedicine appointments. In person visits are currently paused, and information regarding the resumption of in person care will follow as our new model takes shape. Please also note that, due to this transition, we will be operating with limited hours and a limited scope of services until further notice. We appreciate your patience and understanding as we work through these changes. 

Requesting Your Medical Records

Should you wish to transition to a more traditional practice, we can provide on demand recommendations for outpatient primary care facilities where you can reach out and establish care. If you would like a copy of your medical records, please send your request to admin@annabmccallmdllc.com with “MEDICAL RECORDS” in the subject line. Please note there is a 14 business day turnaround for medical records. You may also retrieve records from your FOLLOW MY HEALTH portal account.

We understand this change may raise questions. We encourage you to reach out to our office so we can address any concerns.
